Internal Medicine practice seeking an office-based Nurse Practitioner. Candidate needed to focus on improving patient health, experience, ensuring high-quality outcomes for patients with chronic diseases. Participates in relevant quality assurance and performance improvement measures. Under indirect supervision and working interdependently with physicians, nurses, and other healthcare providers, provides a broad range of nurse practitioner services following established policies and protocols.

• Provides services to patients including but not limited to ambulatory care, taking health histories, performing assessments, implementing nursing interventions, and conducting health education and counseling.
• Assesses and manages patients with routine, minor, and acute illnesses; performs assessment/data collection in an ongoing and systematic manner, focusing on physiologic, psychological, and cognitive status.
• Develops plans of care based on patient assessment ensuring appropriate follow-up in accordance with established nurse practitioner protocols.
• Provides teaching and emotional support to patients and families.
• Triages and refers patients for further care based on severity of illness and symptoms.
• Supervises student nurse practitioners in clinical setting; provides feedback and assistance.
• Utilizes written guidelines/protocol for clinical decision making and management.
• Utilizes application of assessment, planning, implementation, and evaluation in formulating nursing diagnoses and interventions.
• Prepares timely, legible, and complete documentation of all patient care as provided by law, regulation, and established policy.
• Demonstrates an awareness of and sensitivity to patient/family rights.
• Applies safety principles as identified by established policy.
• Participates in continuing education to continually improve skills and abilities and stay abreast of current technologies/practices.
• Ensures compliance with legal issues including but not limited to patient confidentiality and risk management; ensures compliance with JCAHO, federal, state, and local regulations.
• Participates in quality improvement and utilization review activities.
• Exhibits a high degree of courtesy, tact, and poise when interacting with patients, families, and other healthcare professionals.
• Adjusts to fluctuating peaks in patient flow, acuity, and other operational demands while maintaining quality.
• Performs other related duties as assigned by management.

Position Qualifications:

• Graduate from an accredited school of nursing.
• Current RN and nurse practitioner license and CPR certification.
• Two to four years related experience or equivalent.
• Ability to understand and adhere to established policies, procedures, and protocols.
• Demonstrated competence in reacting to and handling emergencies.
• Able to work collaboratively with multiple health professionals in a busy and complex environment using tact, diplomacy, and discipline.
• Strong charting/documentation skills.
• Commitment to excellence and high standards
• Excellent written and verbal communication skills
• Strong organizational, problem-solving, and analytical skills
• Ability to manage priorities and workflow
• Versatility, flexibility, and a willingness to work within constantly changing priorities with enthusiasm.
• Acute attention to detail

• Strong interpersonal skills.
• Ability to effectively communicate with people at all levels and from various backgrounds.
• Good judgement with the ability to make timely and sound decisions
• Bilingual skills a plus.
